Uncommon Immune-Mediated Disease 10) Polymyositis
1. Etiology
- 대부분 primary한 자가면역질환
- 종양에 의해 속발하기도 함: Paraneoplastic immune-mediated myositis (PNS; paraneoplastic syndrome)
- 특히 ==lymphoma 걸린 Boxers==
- 명확한 기전은 밝혀지지 않았으나, Type Ⅳ delayed type - cytotoxic T cells 관련으로 파악.
- SLE는 T suppressor cell, PM은 cytotosic T cell
- IMPA와 함께 발병 가능 → “canine polyarthritis/myositis syndrome”
2. Clinical signs
Clinical signs (근육 관련!!)
- 운동 기능 감소, weakness
- 보행 이상
- 근육 통증
- 식욕 부진, 혀 위축(잘 못 봤지만, 혀도 근육이니까..)
- 거대식도
- 다발성 근염인 만큼, 저작근 근염도 포함되어 같이 발생할 수 있음.
3. Diagnosis
- CK level이 너무 증가? 근육 관련 문제임을 의심. - 특히 polymyositis를 우선적으로 의심해보자.
- 감염질환의 ruleout이 중요함. (Eosinophil이면 특히 감염체 가능성..)
- Boxer lymphoma와 다발성 근염은 밀접한 연관이 있다.
- Treatment : 저작근염과 비슷함
- Prognosis : good
